Part #1: 【Steps】8 Steps to Complete a Standard Physical Examination

Step 1: Download the Foreigner Physical Examination Form designed by the Chinese Quarantine Authority (Attached at the end of this post). Select an appropriate time to take a physical examination as the result is valid for only 6 months.

Step 2: Print the Foreigner Physical Examination Form on the front and back of the same piece of A4 paper. ( Do not print it into two pieces of paper )

Step 3: Fill in your personal information on the Foreigner Physical Examination Form as same as the information on your passport and affix one of your recent passport-sized photos. The name written on the form should not be abbreviated or omitted.

Step 4: Bring the form to a public hospital to complete all the checkups, explain to the staff, and have the doctors fill in all boxes for you in English only. Let them know that signature of the doctor and date at the end of the form are required, hospital stamps on the applicant’s photo and the end are required as well. Make a necessary correction in the hospital before you leave there if it’s incomplete or has any mistakes. The report should be attached with the negative film for Chest X-ray exams, ECG, and the examination certificates/sheets for laboratory exams (Serodiagnosis), which include exams on Cholera, Yellow fever, Plague, Leprosy, Venereal Disease, Opening lung tuberculosis, AIDS, Psychosis, Liver function, and HBsAg. If the physical check-up is done at a private hospital, you should get the certificate notarized.

Step 7: Scan the Foreigner Physical Examination Form, ECG, X-ray, laboratory exam report and other all testing reports into one PDF in order with the Foreigner Physical Examination Form on the first two pages, then submit the PDF to the Chinese university with other required documents. 2. The date of the medical examination is not indicated at the end of the form.

3. The doctor's signature or suggestion is missing.

4. There is no official seal of the hospital, or only have the personal seal of the doctor there. 

5. The student's ID photo is not stamped with the official seal of the hospital, or the stamp is not the same as the second-page ones. 

6. The medical examination form is beyond the validity period of six months.

7. The student's blood type has not been filled in. 

8. The student's home address has not been filled in.

Q1:
My place is locked down due to the COVID-19 pandemic and the hospitals are not approachable. Do I still have to submit a medical report for a Chinese university application? 

 A: Yes. Generally speaking, you can submit a statement first requesting permission of the late submission of the Physical Examination Report from the school. If you don't have a way to communicate with the school for permission and there are still months before the application deadline, then I suggest you wait until the physical examination is available, and submit your application with complete documents. If the application deadline is approaching, then I suggest you submit other materials first and supplement your medical examination form later. If you are applying for Chinese universities through an agency, it will be easy for your agent to communicate with the school about the delay of some of your documents so you don’t have to worry too much. Q2: If I am in China and I have a valid Medical Report in another format from the local Chinese Inspection and Quarantine Center / Chinese International Travel Health Center, do I still have to submit a new medical report in common format when applying for further study?

A: No. According to our experience, the medical report issued by the Chinese Inspection and Quarantine Center / Chinese International Travel Health Center is equally valid when applying for a Chinese university if it does not expire. Normally, you don't have to have a physical examination again. 

 

Q3: I was admitted by a Chinese university in 2020, but I haven't been to China because of the COVID-19 Pandemic and now my physical examination form has expired. Do I need to re-provide a new physical examination report when I can enter China in the future?
A: Yes. The Physical Examination Form is one of the documents for visa processing. Usually, when the school needs to make the JW Form (Visa Form) for you, a valid medical examination report been done within 6 months is required. If the school needs your new medical form, the school will let you know.

 

Q4: What will happen to me if I had a new physical examination in China when I arrive at school and the physical examination report show that my body is abnormal?
A: For students who will study in China for more than six months, their visas will only be valid for 30 days when they enter China. Students need to go to the local Chinese immigration office within 30 days to apply for a residence permit. The immigration office will decide whether you can get a residence permit based on your new medical report. If your physical condition doesn't allow you to get a residence permit, then you can't get a residence permit and stay in China. Conversely, if there is no big problem with your medical report and you have successfully obtained a residence permit, then you can continue to study there. 

 

Q5: The study duration of the program I want to apply for is less than six months. Do I still need to provide the medical form?
A: No. Students who are gonna study for less than six months generally do not need to provide a physical examination. But for double confirmation, please consult the university you apply for.

 

Q6: In my first summer vacation after studying in China for one year, I returned to my country with my residence permit valid. When I go back to China after the holiday, do I need to provide a new medical examination form again? 
A: No. Under normal conditions, students who already have a residence permit do not need to have another physical examination when they enter China again during the study period.

 

Q7: To improve my Chinese, I want to apply for an online Chinese course from a Chinese university and I have no intention of going to China, do I need to submit a medical report?
